چکیده

OBJECTIVES To employ a simple and fast method to evaluate those patients with neurological deficits and misplaced screws in relatively safe lumbosacral spine, and to determine if it is necessary to undertake revision surgery. METHODS A total of 316 patients were treated by fixation of lumbar and lumbosacral transpedicle screws at our institution from January 2011 to December 2012. We designed the criteria for post-operative revision scores of pedicle screw malpositioning (PRSPSM) in the lumbosacral canal. We recommend the revision of the misplaced pedicle screw in patients with PRSPSM = 5' as early as possible. However, patients with PRSPSM < 5' need to follow the next consecutive assessment procedures. A total of 15 patients were included according to at least three-stage follow-up. RESULTS Five patients with neurological complications (PRSPSM = 5') underwent revision surgery at an early stage. The other ten patients with PRSPSM < 5' were treated by conservative methods for seven days. At three-month follow-up, only one patient showed delayed onset of neurological complications (PRSPSM 7') while refusing revision. Seven months later, PRSPSM decreased to 3' with complete rehabilitation. CONCLUSIONS This study highlights the significance of consecutively dynamic assessments of PRSPSMs, which are unlike previous implementations based on purely anatomical assessment or early onset of neurological deficits.and also confirms our hypothesis that patients with early neurological complications may not need revision procedures in the relatively broad margin of the lumbosacral canal.Cite this article: X-J. Surgeon-interpreted Intra-operative Electromyography (EMG) versus Conventional EMG Pedicle Screw Testing—A Prospective Comparison

The reported incidence of lumbar pedicle screw malpositioning is between 5 and 40%. Post-operative neural deficits related to screw misplacement have been reported at rates between 3 and 11%. Radiography alone is an unreliable tool for determining pedicle wall violation due to malpositioned screws, whether by intra-operative fluoroscopy, plain X-ray, imageguided surgery systems based on radiogr...

PediGuardTM: A Solution for the Challenges of Pedicle Screw Placement

Much of the variation in the literature depends on the method used to determine the perforation. Studies using a postoperative CT scan reviewed by independent or blinded reviewers show higher rates of perforation than those determined by radiograph. Laine, et al. reviewed 30 low back operations. In this series of 152 pedicle screws, 32 screw perforations (21%) were detected by CT scan, whereas ...

Complex lumbosacral fracture-dislocation with pelvic ring disruption and vertical shear sacral fracture: a case report of late presentation and review of the literature.

Combination of lumbosacral junction injury with closed vertical shear sacral fracture and disruption of the symphysis pubis is a very rare pattern of injury, particularly with a late presentation. To our knowledge, the complexity of such a lumbosacral injury with pelvic fractures, which was presented with a chronic condition, has never been addressed or identified in the previous literature. We...

Intraoperative electromyographic monitoring to optimize safe lumbar pedicle screw placement – a retrospective analysis

BACKGROUND The foremost concern of a surgeon during pedicle screw fixation is safety. Assistive modalities, especially intraoperative electromyographic monitoring (EMG) can function as an essential tool to recognize screw malposition that compromise neural integrity, so that the screws can be repositioned immediately rather than later. We intend to study the efficacy of intraoperative EMG monit...

Pneumomediastinum, Subcutaneous Emphysema, and Tracheal Tear in the Early Postoperative Period of Spinal Surgery in a Paraplegic Achondroplastic Dwarf

Achondroplasia was first described in 1878 and is the most common form of human skeletal dysplasia. Spinal manifestations include thoracolumbar kyphosis, foramen magnum, and spinal stenosis. Progressive kyphosis can result in spinal cord compression and paraplegia due to the reduced size of spinal canal. The deficits are typically progressive, presenting as an insidious onset of paresthesia, fo...
